Aged Care

Paterson Healthcare provides general practice services to support the health needs of older adults in the community and residential aged care settings. Care is delivered by independent doctors and focuses on appropriate medical assessment, continuity of care, and coordination with carers, nursing staff, and allied health providers where relevant.

Aged Health Assessments

Aged health assessments are available to support the review of physical health, mental wellbeing, and functional capacity in older patients. Assessments are based on individual clinical needs and may include review of medical history, current health concerns, medications, and lifestyle factors. These assessments assist doctors in identifying health risks, monitoring existing conditions, and planning appropriate ongoing care.

Preventive Health Care

Preventive health services may be provided as part of routine general practice care to support early identification and monitoring of health concerns. Preventive care may include assessments related to: Skin health Eye health, including glaucoma risk Bone health, including osteoporosis risk Bowel health Diabetes monitoring Services are offered based on clinical assessment and current guidelines.

Healthy Lifestyle Reviews

Healthy lifestyle reviews may assist patients in understanding key health indicators and risk factors. These reviews may include: Blood pressure checks Blood glucose monitoring Cholesterol assessment Advice and follow-up are provided where clinically appropriate.

Immunisations for Older Adults

Immunisations may be provided in line with the National Immunisation Program and eligibility criteria. Vaccinations may include: Annual influenza vaccination Pneumococcal vaccination Shingles vaccination (subject to age and eligibility requirements) Eligibility and funding depend on individual circumstances and Medicare criteria.

Advance Care Planning

Advance care planning discussions may be offered to support patients in documenting their preferences for future medical care. These discussions are voluntary and based on individual needs and circumstances.

Palliative Care Support

Patients with life-limiting illnesses may be supported through palliative care services provided by independent doctors, in collaboration with specialist teams and community services where appropriate.

Nursing Home Visits

Doctors from Paterson Healthcare may provide visits to local residential aged care facilities to support ongoing medical care. Services are provided in coordination with facility staff and relevant healthcare providers.

Multidisciplinary Care

Where appropriate, doctors may participate in multidisciplinary case conferences to support coordinated care for patients with complex medical needs, including mental health and chronic conditions.

Medication Management Reviews

Medication management reviews may be arranged to assess current medicines and identify issues related to use, safety, or effectiveness. Reviews are conducted by doctors and may involve coordination with the patient’s usual pharmacy, including home-based consultations where eligible.

Health Assessments for Patients Aged 75 Years and Over

Patients aged 75 years and over may be eligible for an annual health assessment conducted by an independent doctor. The assessment allows time to discuss health concerns, daily functioning, and ongoing care needs, with advice, monitoring, or referrals provided where clinically indicated.

Health Assessments for Residents of Residential Aged Care Facilities

Health assessments for residents of aged care facilities may be undertaken to review overall health and functional status. These assessments may include consideration of: Mobility and daily functioning Cognitive health Pain management Skin integrity and foot care Continence Hearing, vision, and oral health The purpose is to support appropriate care planning and ongoing management.

Coordinated Veterans’ Care (CVC) Program

Eligible veterans may be supported through the Coordinated Veterans’ Care Program, where applicable. This program involves collaboration between the patient’s usual GP and other healthcare providers to assist with the management of chronic conditions, regular review, and coordination of care.
