Medication Management Reviews
Medication management reviews may be arranged to assess current medicines and identify issues related to use, safety, or effectiveness. Reviews are conducted by doctors and may involve coordination with the patient’s usual pharmacy, including home-based consultations where eligible.
Health Assessments for Patients Aged 75 Years and Over
Patients aged 75 years and over may be eligible for an annual health assessment conducted by an independent doctor. The assessment allows time to discuss health concerns, daily functioning, and ongoing care needs, with advice, monitoring, or referrals provided where clinically indicated.
Health Assessments for Residents of Residential Aged Care Facilities
Health assessments for residents of aged care facilities may be undertaken to review overall health and functional status. These assessments may include consideration of: Mobility and daily functioning Cognitive health Pain management Skin integrity and foot care Continence Hearing, vision, and oral health The purpose is to support appropriate care planning and ongoing management.
Coordinated Veterans’ Care (CVC) Program
Eligible veterans may be supported through the Coordinated Veterans’ Care Program, where applicable. This program involves collaboration between the patient’s usual GP and other healthcare providers to assist with the management of chronic conditions, regular review, and coordination of care.
