As part of this process, your primary care doctor (PCP) will collaborate with your chosen community pharmacy for convenient at-home consultations.

Health Check for people aged 75 and older (annual) (approx. 60 – 90 minutes)

If you're 75 or older, you should avail our yearly Health Checkup specifically designed for you. During this session, our independent and experienced doctors will spend 60 to 90 minutes discussing your health concerns with you. Together, we will work to find solutions that enhance your health and overall quality of life.

Health Check for Residents of Residential Aged Care Facilities

We provide health assessments for seniors designed to identify preventable health issues, ensuring older individuals receive the necessary support for improved well-being. Our assessments encompass daily functioning, cognitive abilities, pain management, foot care, skin health, continence, hearing, vision, and dental care to enhance quality of life."

Coordinated Veterans' Care (CVC) Program (approx. 60 – 90 minutes)

The CVC Programme provides specialized support to Gold Card holders with chronic diseases, aiming to prevent avoidable hospitalizations. Our approach involves collaborating with your primary care team to enhance condition management, coordinate care, and ensure regular monitoring. You will receive a personalized care plan to track treatments, appointments, and wellness goals.
